Rates listed are for private pay/out-of-pocket. Currently, private insurance does not offer coverage for peer support services with Fusion Collaborative LLC.

credit/debit card; personal check; cash; kittens (kidding...)
Fusion Collaborative is a contracted provider with Sheboygan County Health & Human Services, (including via CCS, CSP, Outpatient Services, and other programs).
If you are receiving certain services through SCHHS, you may be able to access our services at no cost to you.
